Navigator for Youth Transitioning to Adult Services

High School Graduation is a traditional time of transition, However, for Youth living with CP and other disabilities, this transition is much more complicated as the graduate is no longer eligible for many programs and supports provided through Children’s’ Services. Youth with Disabilities and their families must find their way through an often-bewildering array of adult services to arrange the supports that they need.

The CPABC Navigator for Youth in Transition to Adult Services is here to help you connect with the services they need through our specialized information and referral resource. The Navigator service is available for youth aged 14 to 25, their parents and members of their Transition Support Teams.
